THE
NEW REGULATIONS WILL AFFECT EVERY BREATH ALCOHOL TEST CONDUCTED
IN ARIZONA AFTER THIS DATE AND TIME!
As
of 9:00 a.m. on May 18, 2006, utilize these new DPS forms for most
breath alcohol testing:

but have new exhibit letters referenced by the new regulations.
Additional forms listed in the rules may be needed depending on
individual agency programs.
Do
not use any forms which contain the line "Arizona Department of
Health Services"
